Process safety consultant in Brisbane: HAZOP facilitation, management of change and mechanical integrity

We run the hazard studies, risk workshops and change processes that keep an operating plant safe and defensible. The facilitator is a trained HAZOP leader and a chartered mechanical engineer who has directed statutory compliance and management of change on gas-field water treatment and power facilities.

Who we work with

Operations and engineering managers at water, wastewater, desalination, gas and industrial facilities; project teams that need an independent facilitator for a design review; owners who need a management of change system that works in practice.

Problems we are asked to solve

  • A design change or upgrade needs a HAZOP or HAZID with an independent facilitator and a clean actions register.
  • Plant modifications happen without a change process, and nobody can show what was assessed.
  • Pressure equipment and other statutory items lack an inspection program or the records to prove compliance.
  • Safety procedures have drifted from how the plant is actually run.
  • An incident needs an investigation that finds causes rather than culprits.

What we do

  • HAZOP and HAZID facilitation at concept, detailed design and pre-commissioning stages
  • Risk assessment workshops: operational, business and project risk, using your risk matrix
  • Management of change: system set-up, procedures, reviews and audits
  • Mechanical integrity programs, including pressure equipment in-service inspection to AS/NZS 3788 and statutory registration
  • Review and improvement of safety procedures, permits and isolation practices
  • Incident and failure investigation using ICAM and Apollo root cause analysis
  • Technical assurance and engineering governance for operating sites

Recent work

  • HAZOP facilitation for a chloramination upgrade at a Victorian water treatment plant, at both concept and detailed design stages.
  • Statutory compliance program for pressure equipment integrity across gas-field water treatment facilities in Queensland, including two large reverse osmosis plants and a gas-fired power station.
  • Management of change for plant modifications, PLC and SCADA upgrades and reliability improvements at the same facilities.
  • ICAM investigation of a pipe failure on a reverse osmosis high-pressure pump at a seawater desalination plant.

Questions we are often asked

How long does a HAZOP take?

A single process unit or upgrade is usually two to four workshop days plus report writing. We scope it from your P&IDs before quoting.

Do you bring the scribe?

Yes. We facilitate and record the workshop, and issue the worksheets within a week of the last session.

Can you set up management of change for a small site?

Yes. The procedure is scaled to the site: a one-page form and a register are often enough, provided the review step is real.
